*Registration fee is nonrefundable.

**For an accepted paper to appear in the proceedings, at least one author must register. Separate registrations are required for separate papers. Therefore, if an author has two accepted papers, one of the co-authors must register for the second paper. Registration fee includes the electronic proceedings, all coffee breaks, the reception and the conference banquet. Extra banquet ticket can be purchased at the conference desk during the conference. Other meals are not included in the registration fee. Registration must be accompanied by payment. Just mailing this form does not constitute registration.

***Student registration includes the electronic proceedings but not include the banquet. A registered student can attend the SEKE 2015 conference, all workshops and coffee breaks. Banquet tickets may be purchased for an extra charge.

****Regular papers are limited to SIX pages, and short papers are limited to FOUR pages. If approved by conference chair, authors may use this form to pay for TWO EXTRA pages. Posters are not considered accepted papers but are given ONE page in the proceedings.

#Authors unable to attend the conference must arrange for a co-author or a colleague to present the paper and notify the conference secreariat before May 10, 2015. Registration fee is nonrefundable.

##International authors may request an official invitation letter from the conference secretariat. Please provide fax number of US consulate in your country and submit visa application two months prior to conference.

The undersigned, desiring to publish the above paper in a publication of the KSI or cosponsored by the KSI hereby transfer their copyrights in the above paper to the Knowledge Systems Institute, known as the KSI.

Returned Rights: In return for these rights, the KSI hereby grants to the above authors, and the employers for whom the work was performed, royalty-free permission to:
1. Retain all proprietary rights other than copyright, such as patent rights.
2. Reuse all or portions of the above paper in other works.
3. Reproduce, or have reproduced the above paper for the author's personal use or for company use provided that (a) the source and KSI copyright are included, (b) the copies are not used in a way that implies KSI endorsement of a product or service of an employer, and (c) the copies per se are not offered for sale.
4. Make limited distribution of all or portion of the above paper prior to publication.
5. In the case of work performed under U.S. Government contract, the KSI grants U.S. Government royalty-free permission to reproduce all or portions of the above paper, and to authorize others to do so, for U.S. Government purposes.
